If you are a new patient, please book an "Initial Assessment". Intake forms will be emailed to you to fill out prior to your appointment. If you are an existing patient who has not seen the doctor for more than a year or if you have a new complaint that would need extra time to assess properly, please book "Re-assessment" Book "Treatment" for a regular 15 -20 minute visit. Book "Extended Treatment" for a longer 30 minute visit. Due to COVID, please arrive no more than 5 minutes before your appointment time. A COVID screening questionnaire will be emailed to you a couple of hours prior to your appointment and will need to be filled out. A mask must be worn when inside the clinic. If you do not have a mask, one will be available for a cost of $2.00. If you need to cancel an appointment email jspencer@chirodoc.ca or call the clinic directly. Missed or cancelled appointments without 24 hours notice may be subjected to a fee.
